Fees and insurance

PSYCHOTHERAPY FEES

In Spring 2021, I resigned from my contracts with BCBS and Aetna. I am currently an out-of-network provider. Clients pay me directly by cash, check, or credit card at the time of their psychotherapy sessions.  I provide my clients who request this service with monthly superbills that include service dates and billing and diagnostic codes that can be submitted (by clients) to their insurance companies for potential reimbursement.  My fee is $185 for psychotherapy appointments, with the exception of the initial intake session, which is $225. If you are curious about typical costs for such services, please review the following website:

Codes for individual psychotherapy and initial sessions are 90837 and 90791, respectively, and my practice zip code is 85258.

You might receive reimbursement from your insurance company for services, but I can in no way guarantee this; out-of-network coverage varies widely by insurance plan. Please call your insurance coverage and ask about out-of-network coverage if you are interested in potential reimbursement. There might be an out-of-network deductible you would need to meet before they provide reimbursement.

LATE CANCELLATIONS AND NO SHOWS

Please note that I charge a $150 fee for late cancellations (within 24 hours of session time) and no shows. Insurance does not reimburse for this, so you are responsible for these payments. I would much rather be paid to actually see you! Please be thoughtful in scheduling so I can ensure all clients are able to be seen for sessions so we can make progress together.

FEES FOR OTHER SERVICES

Psychotherapy is the primary service I provide in my practice. I do not currently offer psychoeducational assessments and I do not do psychological evaluations for the purpose of writing letters (e.g., for surgeries, FMLA, accommodations). Sometimes, however, during the course of therapy, clients have needs for some additional services outside of therapy, such as my consulting with their school, psychiatric provider, attorney, etc. or completing paperwork. Insurance does not cover these services, which I charge at a rate of $185/hr in 15 minute increments (e.g., if I spend 30 minutes on a consultation, it would cost $92.50). We will discuss any such services and the associated expected charge prior to my engaging in any of these services.

Please note that there is a $25 record fee for my reviewing and copying your file to provide elsewhere.
